I don't anymore, but before I came to Hadley I was a serious cocaine addict. In my case, I was a "mainliner."

Have you been diagnosed with any psychiatric issues? If so, please list diagnoses and age of diagnoses below:
When I first got arrested, my folks took me to a shrink who said I had a casual attitude towards sex and a cocaine addiction. I don't know if either of those are psychiatric issues, but they sure got me in a lot of trouble. I was twelve years old at the time.

If you answered yes to the above, please list any and all prescriptions, including dosage and frequency of dosage, and other treatment plans below:
I was on some pill or another when I was at Hadley--God alone knows what it actually was--twice a day. But before that, I didn't take any prescription meds.

If you answered yes to the above, please describe noted feelings, symptoms, triggers and incidents associated with the diagnoses, and ages of incidents below:
This is about the coke addiction? Well, I had a lot of feelings while I was on it, but the most intense was a feeling of euphoria. For a few minutes while I was high off of it, I felt like God. Physically, around the last couple of months I was actually on it, I noticed that my heart sometimes skipped around and beat irregularly, but I still don't know if that was emotionally caused or a symptom of the coke.

Have you ever been hospitalised for these diagnoses? If yes, please provide facility name(s) and age of admissions and discharges:
Yes, after my third arrest I was sent to the Hadley Institute for Troubled Youth, around December 15, 2010, age fourteen (almost fifteen). I remained there for five months until the closing of the facility and forced move.

Are you aware of a history of psychiatric complaints in your family? If yes, please list family member(s), diagnoses, and how this affected you personally:
My parents aren't hovering over my shoulder this time, so I can say this--they're cultists. I don't know if that's strictly speaking a psychiatric complaint, but the hive-mind mentality sure as hell affected me. First of all, it affected the way I was raised; I grew up attending church-sponsored functions almost every day, to say nothing of the three-hour church services on Sundays. Second of all, it drove me to the cocaine addiction--I needed an escape.

I was a scheduled C-section. There were no issues.

Are you aware of any difficulties in development during your infancy, such as learning disabilities, speech impediments, etc? If yes, please explain below:
I wasn't particularly talkative until I was almost two years old. I mean, I didn't even babble in baby talk.

Are you aware of any long term separation from your mother during your infancy? If yes, please explain below:
Of course not. Our church taught that a woman's role was to be a mother and wife.

Please describe the location and environment in which you grew up. Please list town and state, and what it was like and how this affected you:
I was born and raised in Red Hook, New York. My early childhood was okay, I guess, but as I mentioned earlier, I spent almost all my free time at church. I grew up with two older siblings, twins, Agatha and Jackson. They're nineteen now. Aggie is the perfect little cultist--she's just like our parents, so I don't spend any time with her if I can help it. Jackson is my best friend and my confidant. He got thrown out of the house when he was seventeen and I was fourteen because they found out he was bi, into S&M, and...sick. (Like ill, not like twisted.) I spent a lot of time in church as I got older and my parents found out the stuff I was into (twelve and up) being prayed over and generally churched at. It almost turned me off Christianity, but I found God my own way when I was thirteen and it's helped me a lot. Disobedience was quickly and summarily punished, from the time I can remember, with a beating. I can still quote Proverbs 22:15 from memory: "Foolishness is bound in the heart of a child; but the rod of correction shall drive it far from him." Father used to quote it right before he commenced with the punishment. I can truthfully say that he never laid a hand on me--he always used either a cane or a wooden paddle.

I was always a mostly straight-A student. If I got anything less, my father would beat me nightly until I brought home proof that the grades had gone up.

Do/did you have many friends growing up? Do/did you have a best friend? If yes, please explain below:
As a young child, I was only permitted to associate with children who attended our church, and I didn't care for any of them because they were all automoton robots. Jackson and I were super-close. When I was eleven, I met Henry. Henry introduced me to Teddy, Teddy introduced us to Peter, and Peter introduced us to cocaine.

Do/did you pursue any extra curricular activities or talents? If yes, please explain below:
In elementary school, as I mentioned, I was involved in a lot of church-based activities. I also took piano lessons. When I got older, my "extracurricular activities" were basically limited to sex, drugs, and--well, okay, classical music rather than rock and roll, but you get the idea.

If sexually active, please list age of onset and sexual preference below. If not sexually active, are you aware of a sexual preference?:
I've been sexually active since I was eleven and a half. I am gay, which I have known since I was very small, but I have been known to have sex with women--if the price was right.

Yes. As I've mentioned several times on this application, the whole reason I'm here is because I'm a cocaine user. I swear by all that is holy, however, I have never done any drugs other than cocaine.

Have you ever been convicted of a crime? If yes, please list conviction, age of conviction and circumstances below:
Three arrests for Possession With Intent, two for prostitution, one for Public Indecency. The first arrest, at age twelve, was when the police caught me, Henry, Peter, and Teddy shooting up in Peter's basement; that was for Possession With Intent because of the amount of cocaine we had. The second time, at age fourteen, I made the mistake of soliciting an undercover cop--I had started prostituting myself at twelve to pay for my drug habit--and was arrested for prostitution as well as PWI, since I had about an ounce of cocaine on me. The third time I got arrested, right before I got admitted to Hadley, was PWI, prostitution, and Public Indecency. This guy had come up to me after school and offered me quite a large amount of cocaine in exchange for sex, if I did it exactly the way he wanted. Usually I just got money, but I figured this way I wouldn't have to go out so much that night, so I agreed. We ended up having sex on a trampoline in his backyard. It was after dark, but the police caught us anyway. I was, as I mentioned, fifteen, nearly sixteen.

The first time I was given a warning about the dangers of drugs and let go. The second time I had to spend thirty days in a court-ordered detox program. The third time I was sentenced to juvenile detention and probably would have been there until I turned eighteen if it hadn't been for Hadley.

Have you ever knowingly committed a crime for which you were not convicted or sentenced? If yes, please explain below:
I only got arrested three times, but I did drugs way more than that and I did a lot of prostituting.

I wouldn't. I don't believe in predestination and all that crap, but I do believe things happen for a reason. Maybe there's a reason God wanted me at Hadley.

How do you feel about your life and the things you have done in it, looking back? If you have committed crimes, how do you feel about those now?:
I regret abusing cocaine for so long, but honestly I can't bring myself to feel bad about being a prostitute, or being gay for that matter.

What are your goals in life, and how do you feel those would be best accomplished?:
Actually, I want to be on Broadway or in the opera someday. I think the best way for me to do that is to kick my drug habit, become a productive member of society, and get the hell out of here.

Lastly, why have you chosen admission at St. Christina Psychiatric Rehabilitation Center, and how do you think this will help you toward your life goals?:
I didn't--I was at Hadley, and we weren't given a choice, just like I wasn't given a choice about going to Hadley. But I think I would have chosen it if I could have, because Hadley has done a surprising amount to help me and I can only pray that your facility will augment that.
