I began having migraines at the age of 14. The doctor I saw at the time said it was a heredity thing, because my mother has had them from a young age too, and both of her parents before her.

N/A. I take over-the-counter medication for the migraines as needed.

I'm a social smoker. It's not something I do all the time. On most days I don't smoke at all, but the most I have ever smoked in any given night was seven cigarettes, if I recall correctly. Likewise, I like to drink in casual and/or social situations, but I don't abuse alcohol. I have never found myself in the throes of addiction to a substance. I've never caught myself thinking that I've "gotta have" anything. I'm fortunate in that regard.

Besides having been among the top of my graduating class in university, I'm a very gentle, compassionate individual, and I pride myself on indiscriminately treating individuals with the utmost of care. I'm a great listener and a fantastic caregiver. I'm flexible with my schedule, a quick learner, courteous, conscientious, punctual, agreeable and fair. I am willing to do whatever the job requires of me, and I'll do it with a smile on my face, which, if you ask anyone who knows me well, is not an exaggeration in the least. Taking care of people is what I live for, and you will never see me hesitate to lend someone a hand.

I interned at Tulane Medical Center as I was earning my Bachelor's degree at the university and working at Shady Grove. From doing this, I learned that I possess seemingly endless reserves of energy and excellent time management skills. I attended class in the mornings, went to my internship in the afternoons, and worked for a few hours in the evenings at Shady Grove. I came very close to burning myself out, but the important thing is that I was successful and learned the value of hard work and compassionate care. I can cope with a hectic schedule and I like being busy, which is one of the many reasons why I am well-suited for the medical field.

As you can see, I have plenty of experience and expertise in the medical field. I feel that Hadley especially has given me ample preparation for this position. I had never worked with troubled youths before, but now that I have, I can say with confidence and optimism that I am a good fit for this position and am thoroughly capable of doing the job to the best of my ability.

I'm not addicted to alcohol, but I have abused it in the past. Partying is a way of life in New Orleans, and I admit that I went overboard a time or two. Many of my weekends were spent with horrible hangovers. I have tried marijuana and ecstasy in the past, but I have not indulged in either in over two years. Now, I drink occasionally for recreational and social enjoyment, but I certainly don't abuse alcohol the way I used to.

I haven't. It's no secret that I love to go out and live it up on the weekends, but I've kept my record as clean as a whistle!

Describe how you would handle a situation in which a patient may become violent towards themselves or others:
At first, I would try to talk to the patient to see if there is anything I can do to help calm him/her down. If the behavior continues to escalate and the patient is in danger of lashing out, I would request backup for help restraining the patient so he/she is a not a danger to him/herself or any others. After tending to any immediate injuries, I would leave it up to the patient's primary therapist to decide what to do with him/her from there. As a nurse, it is not up to me to decide upon punishment for a patient who has acted out.

If your immediate supervisor instructed you to complete a task to which you felt morally opposed, how would you handle the situation?:
If I felt morally opposed to anything I might be asked to do in this position, I would not have applied for it. However, if I was asked to do something I was vehemently against, I would voice my concerns and try to reach some sort of compromise with my superior.

If you discovered a fellow employee breaking facility rules and endangering patients or staff, how would you handle the situation?:
I would report the employee to his or her immediate supervisor and let them decide the proper course of action. Endangering patients or staff is simply not an acceptable practice, and I could not remain quiet about it.
